Lieutenant Commander Vaughan was a member of Heavy Photographic Squadron 61, U.S. Navy. On October 14, 1967, he was the pilot of a Douglas Skywarrior Attack Aircraft (RA-3B) over Hon Me Island, Thanh Hoa Province, North Vietnam in the Gulf of Tonkin when his aircraft crashed. His remains were not recovered. His name is inscribed on the Courts of the Missing at the Honolulu Memorial.
