Lieutenant Commander Tapp was a member of Attack Squadron 93, Carrier Air Wing 9 aboard the Aircraft Carrier USS ENTERPRISE (CVAN-65). On March 23, 1966, he was the pilot of a Douglas Attack Aircraft Skyhawk (A-4C) over North Vietnam. As he returned to the USS ENTERPRISE, about 8 miles away, contact was lost. His remains were not recovered. His name is inscribed on the Courts of the Missing at the Honolulu Memorial.
