Lieutenant Colonel Smith was a member of the 8th Bomber Squadron. On March 11, 1965, he was the co-pilot of a Martin Canberra Bomber (B-57) on a bombing mission about 30 miles northeast of Kontum near the border of Kontum and Binh Dinh Provinces in South Vietnam, when his aircraft was shot down. His remains were recovered on February 7, 1994 and identified on September 6, 1994. His name is inscribed on the Courts of the Missing at the Honolulu Memorial.
