Lieutenant Commander Perry was a member of Attack Squadron 164, Carrier Air Wing 15 aboard the Aircraft Carrier USS ORISKANY (CVA-34). On August 31, 1967, he was the pilot of a Douglas Attack Aircraft Skyhawk (A-4E) on a raid on Haiphong, North Vietnam when his aircraft was hit by a surface to air missile. He managed to parachute over water but could not be saved. His remains were recovered on November 26, 1986 and identified on February 19, 1987. His name is inscribed on the Courts of the Missing at the Honolulu Memorial.
