Colonel Maxwell was a member of the 357th Tactical Fighter Squadron. On September 12, 1968, he was the pilot of a Thunderchief Fighter (F-105D) on a mission about 5 miles west of Ron, Quang Binh Province, North Vietnam, when his aircraft was shot down. His remains were recovered on June 21, 1989 and identified on October 19, 1989. His name is inscribed on the Courts of the Missing at the Honolulu Memorial.
