Lieutenant Junior Grade Madsen, U.S. Naval Reserve, was a member of Attack Squadron 52, Carrier Air Wing 19 aboard the Aircraft Carrier USS TICONDEROGA (CVA-14). On January 18, 1967, he was the pilot of a Douglas Attack Skyraider (A-1H) on a combat mission over North Vietnam. When he returned to the carrier he was not able to reduce the speed of the aircraft to landing speed, circled the carrier and crashed into the sea. His remains were not recovered. His name is inscribed on the Courts of the Missing at the Honolulu Memorial.
