Notes
Commander LaHaye was a member of Fighter Squadron 111, Carrier Air Wing 2 aboard the Aircraft Carrier USS MIDWAY (CVA-41). On May 8, 1965, serving as commanding officer of Fighter Squadron 111, he was the pilot of a Vought Crusader Fighter (F-8D) over North Vietnam. Leading a flight of eight aircraft on an antiaircraft suppression mission against an enemy airfield, CDR LaHaye, in the face of heavy, hostile antiaircraft fire, initiated the strike for Attack Carrier Air Wing Two and struck his target with devastating effect. When his aircraft sustained severe damage from the antiaircraft fire, he piloted the stricken jet through intense opposition away from enemy territory to the open sea. His remains were not recovered. His name is inscribed on the Courts of the Missing at the Honolulu Memorial.
