Commander Kollman was the commanding officer of Attack Squadron 35, Carrier Air Wing 0 aboard the Aircraft Carrier USS ENTERPRISE (CVAN-65). On March 12, 1968, he was the pilot of a Grumman Attack Aircraft Intruder (A-6A) going on a night mission. The catapult launch malfunctioned and the aircraft ditched right off the catapult. His remains were not recovered. His name is inscribed on the Courts of the Missing at the Honolulu Memorial.
