Corporal Hoffman was a member of the Marine Medium Helicopter Squadron 262, Marine Air Group 39, 1st Marine Air Wing. On August 19, 1968, he was flying in a Boeing Sea Knight Cargo Helicopter (CH-46D) over Quang Tri Province, South Vietnam. His aircraft was hit by enemy fire and crashed. His remains were recovered on August 3, 1993 and identified on April 19, 1994. His name is inscribed on the Courts of the Missing at the Honolulu Memorial.
