Lieutenant Foulks, U.S. Naval Reserve, was a member of Attack Squadron 163, Carrier Air Wing 16 aboard the Aircraft Carrier USS ORISKANY (CVA-34). On January 5, 1968, he was the pilot of a Douglas Attack Aircraft Skyhawk (A-4E) on a night reconnaissance mission over North Vietnam. Contact was lost when his aircraft was at Phat Diem, Ninh Binh Province, North Vietnam. His remains were recovered on December 15, 1988 and identified on January 12, 1993. His name is inscribed on the Courts of the Missing at the Honolulu Memorial.
