Captain Duffy was a member of the 20th Tactical Air Support Squadron. On April 4, 1970, he was the pilot of a Cessna Skymaster Observation Aircraft (O-2A) on a mission about 15 miles southwest Quang Ngai, South Vietnam, where it was shot down. His remains were recovered on June 8, 1993 and identified on February 12, 1996. His name is inscribed on the Courts of the Missing at the Honolulu Memorial.
