Colonel Cook was a member of the 16th Tactical Reconnaissance Squadron, Tan San Nhut Air Base, South Vietnam. On April 28, 1968, he was the pilot of a McDonnell Phantom II Reconnaissance Fighter (RF-4C) on a photographic mission into Quang Tin Province about 20 miles west-southwest on Chu Lai, South Vietnam. Radio contact was lost. His remains were not recovered. His name is inscribed on the Courts of the Missing at the Honolulu Memorial.
