Lieutenant Colonel Cook was a member of Communication Company, Headquarters Battalion, 3rd Marine Division. While acting as an advisor to the 4th Battalion, Vietnamese Marine Corps, he was taken prisoner in the Delta of South Vietnam. On December 31, 1964, he died of Malaria while being transferred from one prison camp to another. His remains were not recovered. His name is inscribed on the Courts of the Missing at the Honolulu Memorial. For his leadership and great valor, Lieutenant Colonel Cook was awarded the Medal of Honor.
