Lieutenant Cook was a member of Attack Squadron 212, Carrier Air Wing 21 aboard the Aircraft Carrier USS HANCOCK (CVA-19) at Dixie Station in the South China Sea. On April 6, 1966, he was the pilot of a Douglas Attack Aircraft Skyhawk (A-4E) preparing to launch for an attack mission when the catapult failed and his aircraft fell into the sea below. His remains were not recovered. His name is inscribed on the Courts of the Missing at the Honolulu Memorial.
