Captain Chavez was a member of the 14th Tactical Reconnaissance Squadron, 432nd Tactical Reconnaissance Wing, Udorn Airfield, Thailand. On July 30, 1970, he was the pilot of a McDonnell Phantom II Reconnaissance Fighter (RF-4C) on a night reconnaissance mission over southern Laos. When contact was lost the aircraft was 25 miles north of Muang Fangdeng, Attopeu. His remains were not recovered. His name is inscribed on the Courts of the Missing at the Honolulu Memorial.
