Lieutenant Commander Callahan was a member of Attack Squadron 106, Carrier Air Wing 10, U.S. Navy aboard the Aircraft Carrier USS INTREPID (CVS-11). On September 23, 1968, he was the pilot of a Douglas Attack Aircraft Skyhawk (A-4E) on a routine mission over North Vietnam when the aircraft's generator failed. On landing the aircraft crashed on the deck and fell into the ocean. His remains were not recovered. His name is inscribed on the Courts of the Missing at the Honolulu Memorial.
